Your health information, protected.
Summit Elite IV & Aesthetics is committed to safeguarding your Protected Health Information (PHI) in full compliance with the Health Insurance Portability and Accountability Act (HIPAA). This page summarizes how we collect, use, protect, and disclose your health information — and the rights you hold over it.
Last updated: September 2026
Our commitment
Your privacy is foundational to the care we provide. The clinical relationship only works when you can share your health history openly, knowing exactly where that information goes. We maintain administrative, physical, and technical safeguards to protect your PHI against unauthorized access, use, disclosure, and loss.
How we use and disclose your health information
We use and disclose PHI only as permitted by HIPAA, including:
- Treatment — coordinating your care with our clinical team, including Good Faith Exams, intake review, and follow-up
- Payment — processing payments and billing for services you request
- Health care operations — quality assurance, compliance oversight, and clinical supervision
- Business associates — vetted service providers who assist our operations under signed agreements protecting your PHI
- As required by law — disclosures to public health authorities or as otherwise legally mandated
Your rights regarding your health information
Under HIPAA, you have the right to:
- Access and receive a copy of your PHI, in paper or electronic form
- Request an amendment to your PHI if you believe it is inaccurate or incomplete
- Receive an accounting of certain disclosures of your PHI
- Request restrictions on certain uses and disclosures of your PHI
- Request confidential communications — for example, at a specific phone number
- Receive a copy of our full Notice of Privacy Practices on request
Our responsibilities
In return, we are required to — and do — maintain the privacy and security of your PHI, notify you in the event of a breach of unsecured PHI, and comply with all HIPAA requirements. We will never retaliate against you for exercising your privacy rights or filing a complaint.
Complaints & questions
If you believe your privacy rights have been violated, we want to hear from you directly. Contact our care team at care@summiteliteiv.com, and we will investigate and respond. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
Important note
This page is a plain-language summary provided for your convenience. It does not replace our full Notice of Privacy Practices, which is available on request and governs our legal obligations. If any conflict appears between this summary and the full Notice, the full Notice controls.
