Dermal Fillers 101: What They Actually Do (and How Long They Last)

Dermal fillers have become one of the most requested aesthetic treatments in the country, and one of the most misunderstood. Search "dermal fillers Nashville" and you'll find everything from med spas to Groupon deals, all promising similar results at very different levels of oversight. As the Medical Director of Summit Elite IV & Aesthetics, I want to give you the same explanation I give every client before their first consultation: what fillers actually do, what they can't do, how long they last, and what separates a safe treatment from a risky one.

What Dermal Fillers Actually Are

Most modern dermal fillers are made of hyaluronic acid (HA) — a sugar molecule your body already produces. HA's natural job is to bind water and add volume, which is why it works so well as an injectable: it restores lost structure under the skin and draws hydration into the area for a plumper, smoother appearance.

Fillers are not Botox. The two are constantly confused. Botox relaxes the muscles that cause dynamic wrinkles — the lines that appear when you frown or squint. Fillers restore volume — the loss that happens as we age, when fat pads in the face shift and descend, and skin produces less collagen. Many clients benefit from both, but they solve different problems, and your consultation should start by identifying which one is actually causing the concern you see in the mirror.

The American Society of Plastic Surgeons offers a good plain-language overview of how these products work and how they differ from neurotoxins.

What Fillers Can Treat

  • Nasolabial folds and marionette lines — the lines running from the nose to the mouth and downward from the mouth, often the first place aging becomes visible
  • Lips — restoring volume, improving symmetry, and defining the lip border
  • Cheeks and mid-face — lifting and re-volumizing where fat loss changes the shape of the face
  • Jawline and chin — adding definition and balancing facial proportions
  • Under-eye hollows — softening the tired look that sleep doesn't fix (a treatment that requires particular precision and the right candidate)

What fillers can't do: they can't stop aging, they can't replace good skin care, and they can't fix skin texture the way resurfacing treatments can. Anyone promising a "liquid facelift" that erases a decade is overselling. What they can do — done well — is restore a rested, refreshed version of you.

How Long Fillers Last: The Honest Answer

This is the question every client asks, and the honest answer is: it depends on the product, the area, and your metabolism.

  • Lips: typically 6 to 9 months — lip tissue moves constantly, so filler breaks down faster
  • Nasolabial folds and marionette lines: 9 to 12 months
  • Cheeks and mid-face: 12 to 18 months — thicker, more structurally robust products last longer here
  • Under the eyes: often the longest-lasting area, frequently 12 months or more

Hyaluronic acid fillers are naturally absorbed by the body over time, which is a feature, not a flaw. The result fades gradually rather than falling off a cliff, and an experienced injector can adjust your look as your face changes rather than committing you to one outcome permanently. The FDA's guidance on dermal fillers is worth reading if you want the regulator's view on safety and what to discuss before treatment.

What Safe Treatment Actually Looks Like

This is the part I'd urge you to read carefully, because in Tennessee — like most states — the level of oversight varies dramatically between providers. A safe filler treatment includes:

  1. A real medical consultation first. Every aesthetic treatment at Summit Elite begins with a consultation — your medical history, medications, and goals reviewed before anything is injected. If a provider offers to inject you the same hour you walk in the door with no clinical questions, that's a flag.
  2. An experienced injector working under medical oversight. Injections are procedure, not product. Placement — knowing facial anatomy, avoiding vessels, respecting proportions — is everything.
  3. Genuine products from legitimate sources. Counterfeit filler is a real problem. Legitimate providers can tell you exactly what product is being used.
  4. A plan, not a sales pitch. A good injector will sometimes tell you that you don't need filler yet, or that a different treatment would serve you better. Our consultation process is built around that honesty — including the option to combine Botox and fillers into a private party setting with friends, where the same clinical standards apply in a more relaxed environment.

What to Expect After Treatment

Expect mild swelling, possible bruising, and tenderness for a few days — these are normal. Results are visible immediately but "settle" over one to two weeks as swelling resolves, which is why we bring clients back for a follow-up before judging the final result. The American Academy of Dermatology has a solid rundown of recovery expectations and rare-but-serious warning signs every client should know before treatment.

You should be able to reach your provider after your appointment. Concierge aesthetics should mean concierge follow-up.

Is a Filler Consultation Right for You?

If you're noticing volume loss in your face, asymmetry in your lips, or a jawline that's lost definition, a consultation is the low-pressure way to learn what's possible — and what we'd recommend you not do. Book a consultation at our Cool Springs Wellness Lounge or through our mobile concierge service across greater Nashville, and bring your questions — we'd rather answer twenty of them than see you guess.
